Swimming Lessons Covid-19 Declaration Form

This is a mandatory requirement for EACH child prior to every batch of lessons they attend.

This will ensure the health and safety of all people interacting with Cavan Regional Health, Sports & Leisure CLG. All parents of children in lessons must complete this declaration form. This information may be used at a later date for contact tracing.

note:

If your child is displaying symptoms or has been in close contact with someone who has COVID-19, they SHOULD NOT ATTEND and adhere to HSE guidelines.

We appreciate in advance your co-operation and patience with these changes.
